Fellowship Application Form Instructions
(1) Please log in with Lobo ID and password at bottom of page to access online application form:
Here’s what you will be adding to the application form:
Personal Information – demographic information. Please include secondary email and A-Number.
Admission Status – Note: You do not have to be admitted at the time of application, but you must be fully admitted to a graduate program at Sul Ross State University to receive fellowship funds.
Degree Program -Indicate the degree that you are seeking, major field of study, and the name of your faculty advisor.
Education -Summarize your educational career to date. If an institution that you attended did not use the 4.0 scale, please convert the GPA to a 4.0 scale. In addition to this summary, all official transcripts must be on file in the Center for Enrollment Services.
Test Scores – For applicants that are required to take either test, official score reports must be on file in the Center for Enrollment Services.
Employment – Provide your employment history. Use an additional page if necessary.

STATEMENT OF INTENT: Write a statement of intent in which you explain what your degree and career objectives are and discuss the contributions to your field and/or society that you hope to make in the future. If you plan to obtain additional degrees, certifications, or advanced training, please indicate that as well. This should be a one-to-two-page, double-spaced document.
RÉSUMÉ: Create a curriculum vitae (i.e. an academic resume) that includes your educational achievements, skills, honors, extracurricular activities, and group memberships. This should be a two-to-four page document.
